The following is a FREE Relationship Self-Audit Tool designed to assess the strength of your organizations six key relationships.

Simply complete each of the six relationship surveys and we will return a customized report that compares your assessment of your organizations' performance to the self-assessment of other organizations.

Please answer each question by checking the box that best reflects your level of agreement or disagreement with each statement.  A guide is at the top of each question area.  The first five choices provide a range from "Strongly Disagree" to "Strongly Agree".   The last choice is "Don't Know".
